Individual Telehealth Sessions

$160/ 50 Minutes

Cancelation/No Show Policy

If you are unable to attend a session, please make sure you cancel or reschedule at least 48 hours in advance. Any late cancellations (less than 48-hour notice) or missed appointments will be charged at the full rate of the session. A card will be asked to be on file at the beginning of services.

EXCESSIVE cancellations/no shows (4 or more) will result in termination of services. In order for therapy to be effective, consistent attendance is necessary.

Should you fail to schedule an appointment for THREE consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, I must consider the professional relationship discontinued.

Payment

Payment will be due at the time of service, at the beginning of the day of session. Acceptable methods of payment are all major credit cards, as well as HSA/HFA cards.

Sliding Scale

Sliding scale fees are based on household income. I reserve a few spots on my caseload for sliding fee clients. I will work with you in order to identify a financial agreement that is sustainable. If you do not believe you can afford my full fee, please contact me so we discuss the options. If I am not able to accommodate your needs I would be happy to refer you to trustworthy therapists in the area.

Insurance

As an out of network provider, I do not bill directly through insurance. I would be happy to provide you with a super bill for you to provide your insurance company for possibility of reimbursement. There are benefits to not billing insurance directly, two of which are privacy and control over the work that we do together. Often times in order for insurances to cover services a diagnosis will need to be made within the first few sessions. I always recommend calling your insurance provider and inquiring about their reimbursement rates for out of network providers prior to scheduling a session with me. If you prefer an in network clinician, I suggest using psychologytoday.com to research which clinician may be right for you.

*Note: Due to COVID-19, all counseling services will be conducted through Telehealth services and through the use of a secure video platform. Be sure to ask your insurance provider specifically about reimbursement of telehealth mental health services.

 Good Faith Estimate Notice

As of January 1, 2022, state-licensed or certified health care providers need to give a Good Faith Estimate of healthcare charges to every new and continuing client who is either uninsured or is not planning to submit a claim to their insurance for the healthcare services they seek.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. Providers must inform all self-pay and uninsured patients that a good faith estimate of charges is available. A good faith estimate of expected charges must be given to the patient within specified time frames (e.g., for services scheduled at least 3 days prior to the appointment date, no later than 1 business day after the date of scheduling). The estimate is not binding. However, patients may challenge a bill if the charges substantially exceed the estimated amount( any amount over $400). If there are changes to the information in the good faith estimate, a new estimate should be provided. The estimate can include anticipated charges for recurring services that are expected to be provided within the next 12 months (e.g., 10-20 psychotherapy sessions). If treatment continues beyond 12 months, the provider must give the patient a new estimate. Please note this is NOT a treatment recommendation, rather just a form created to ensure patients do not receive surprise bills they did not expect.

• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

• Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

• If you receive a session bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

• Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
